The delivery service sends the enquiry; it does not use your email account. Please provide your own contact details and do not include a child’s details or medical information.

FormSubmit states that it retains submissions for 30 days. Read FormSubmit’s privacy information . MediBay Ltd may use the enquiry to respond by phone or email and manage your request. Contact the team about access, correction, deletion or how long it keeps enquiries.

Your contact details are not saved in this website’s browser storage. A temporary session marker records only that the pop-up has been shown. If delivery cannot be confirmed, the form keeps your entries on screen so you can review them; closing it clears them.

Marketing choices

The separate, optional marketing checkbox gives MediBay Ltd permission to email you about i-Track and MediBay™ services, news and events. Accepting the Terms & Conditions alone does not give marketing permission. You can withdraw consent at any time by replying to a marketing email or contacting the MediBay™ team.

Private demonstrations

The interactive patient app is a design preview with fictional profiles. Sending an enquiry does not grant access or create a patient account.

Email enquiries

Email links open your chosen email app. An enquiry is sent only when you choose to send it there. Please use your first email for general questions rather than medical records. Ask the MediBay™ team how your enquiry will be handled.

The clinical service’s privacy notice, consent and security arrangements must be confirmed before real patient information is collected.

What you are registering for

These terms cover registering your interest in i-Track with MediBay Ltd (MediBay™). Registration is optional and free. It does not create a patient account, book an appointment, purchase a service or establish a clinical relationship. The UK service is in development, and its availability may change.

Your request

Please provide your own first name, surname, email address and, optionally, phone number, and select Individual / parent / carer or Professional / partner. You may also include an enquiry topic and a general question. Do not enter a child’s contact details or medical information. When you press Send enquiry, the form sends these details and any enquiry you enter, your acceptance of these terms, your marketing choice and the submission date through FormSubmit for delivery to the MediBay™ team. You do not need to send an email yourself.

FormSubmit processes the submission and states that it retains submissions for 30 days. See FormSubmit’s privacy information . The website does not save your contact details in browser storage. Details received in MediBay™’s inbox are handled by the team; contact us about access, correction or deletion.

Contact about your interest

MediBay Ltd may use your name, phone number, email address and selected interest to respond to and manage your enquiry. Accepting these terms does not give permission for marketing emails.
